These forms are Microsoft Word templates. Please use Microsoft WORD on your desktop, IPad or mobile phone to fill in these forms.

NOTE

Pull-down boxes and date boxes are not supported by Microsoft WORD on IPad or Mobile phones. Please download the  IPad/Phone version if you use the devices to complete the forms.  If you use a desktop computer, please download the desktop version that has pull-down boxes and date selection boxes enabled.

TRAINEES

After you have filled in the necessary fields, please save the form in your own computer or memory stick in Word format before sending it to your trainer/assessor for completion and signing.

TRAINERS

After you have filled in the necessary fields, please type your full name in the signature box and save it in PDF format before sending it back to the trainee for safekeeping (DOPS, MiniCEX and CbD), or to the department head / training supervisor (PaedMSF) for collation, or countersigning (Trainer's report).

Uploading to EPS

Trainer's reports should also be uploaded to the EPS.

DOPS, CbD, MiniCEX and PaedMSF should be kept by the trainee as part of the portfolio. They need not be uploaded to EPS.
